Adherence to a Clinical Guideline for Upper Gastrointestinal Bleeding in a Private Hospital in Entre Ríos, Argentina

Abstract

Introduction: Upper Gastrointestinal Bleeding is a frequent cause of emergency hospital admission, and the implementation of standardized protocols allows for resource optimization and improved care effectiveness. Objective: To evaluate adherence to the 2021 European Society of Gastrointestinal Endoscopy Guideline in patients with non-variceal upper gastrointestinal bleeding admitted to general wards or intensive care units in a private medical center in Entre Ríos between 2022 and 2025. Materials and Methods: Non-experimental, descriptive, cross-sectional, secondary study with access to medical records of patients aged 18 years or older, hospitalized in general wards or intensive care, with diagnosis or suspicion of non-variceal upper gastrointestinal bleeding. Only patients with signed Habeas Data consent were included. Sex, age, type of admission, care sector, time to endoscopy, and compliance with Guideline recommendations were recorded. Data were analyzed using SPSS. Results: A total of 58 adults participated; 63.8% were male, with a mean age of 71.26 years (SD 14.59), and 82.8% were admitted through the emergency department. There was high adherence to initial management and transfusion protocols (70.6%), low application of the Glasgow-Blatchford Score (12.3%), and endoscopy within 24 hours was performed in 42.9% of cases. The mean time to endoscopy was 28.67 hours (SD 21.87). Conclusions: Adherence to the European Guideline was heterogeneous, with limitations in the application of the Glasgow-Blatchford Score and timely endoscopy, suggesting the need to improve the implementation of these recommendations.
