Risk Factors Associated with Sarcopenia in Workers with Type 2 Diabetes Mellitus in Markets in Loja, Ecuador. A Pilot Study

Abstract

Introduction: Sarcopenia is a progressive disorder characterized by the loss of muscle mass and function, particularly among individuals with type 2 diabetes mellitus (T2DM). Objective: To evaluate the risk of sarcopenia among workers with type 2 diabetes mellitus from market centers in Loja during 2025. Methods: An observational analytical cross-sectional study00 was conducted in 46 workers previously diagnosed with T2DM. Sarcopenia was assessed through handgrip strength, gait speed, and body composition measured by bioelectrical impedance analysis, according to EWGSOP2 criteria. Descriptive statistics, analysis of variance, and binary logistic regression were performed. Results: Mean age was 59 ± 13 years and 76.1% were women. Risk of sarcopenia was identified in 28.3% of participants, while 4.4% had sarcopenia or severe sarcopenia. Significant differences were observed in skeletal muscle mass and skeletal muscle mass index according to sarcopenia status (p = 0.006). Skeletal muscle mass showed a protective effect against sarcopenia risk (OR = 0.665; 95% CI: 0.429-0.982). Conclusions: A considerable proportion of workers with diabetes were at risk of sarcopenia. Skeletal muscle mass was inversely associated with this condition, highlighting the importance of body composition assessment in diabetic patients.

    Kylie Nicole Cevallos-Chamba. Estudiante de Medicina de la Universidad Técnica Particular de Loja (UTPL), Ecuador, cursando el internado rotativo (agosto 2026 - agosto 2027). Ha complementado su formación académica con cursos de soporte vital básico (BLS) y soporte vital avanzado en trauma (ATLS), así como su participación en el Simposio de Obesidad. Su interés investigativo se centra en la salud metabólica y las complicaciones musculoesqueléticas asociadas a enfermedades crónicas no transmisibles, como la diabetes mellitus tipo 2 y la sarcopenia.

    Mi trayectoria científica se ha centrado en la investigación y docencia en el campo de las enfermedades metabólicas. Durante mis estudios de Máster en Cirugía de Obesidad, me interesó la relación entre el metabolismo de los lípidos y la resistencia a la insulina en pacientes con obesidad mórbida al igual que las alteraciones genéticas y epigenéticas de estas enfermedades. Formo parte del grupo de investigación Prevención y Promoción de la Salud donde he podido trabajar en proyectos de vinculación enfocados a la detección precoz de las enfermedades metabólicas e implementando estrategias de prevención para nuestra población. He colaborado en la publicación de artículos científicos en revistas de alto impacto, estableciendo colaboraciones con investigadores expertos en el campo del metabolismo en países como Estados Unidos y Argentina. Mis intereses a mediano y largo plazo se centran en extender la investigación de las enfermedades metabólicas a nivel nacional para identificar marcadores tempranos o predictores de las enfermedades metabólicas para ser implementadas como políticas de salud nacional; a la par, busco alternativas para la detección temprana de estas enfermedades que sean sencillas, aplicables y sobre todo accesible a la población.
