New non-pharmacological therapies for erectile dysfunction: current evidence on the use of low-intensity shock waves and platelet-rich plasma (PRP).

Abstract

Introduction: Erectile dysfunction (ED) is a common condition with a significant impact on quality of life. While phosphodiesterase type 5 inhibitors (PDE5i) offer symptomatic relief, they do not address the underlying pathophysiological causes. Regenerative therapies such as low-intensity extracorporeal shockwave therapy (Li-ESWT) and platelet-rich plasma (PRP) have emerged as promising alternatives with potential curative effects.
Method: A narrative literature review was conducted using scientific databases including PubMed, Scopus, UpToDate, and DynaMed. Studies published between 2020 and 2025 were selected, focusing on clinical and preclinical evidence related to PRP and Li-ESWT in the treatment of ED.
Results: Li-ESWT has shown statistically significant improvements in erectile function in patients with mild to moderate ED, as measured by the International Index of Erectile Function (IIEF), with minimal adverse effects and good tolerability. PRP has demonstrated potential in promoting angiogenesis, neurogenesis, and smooth muscle restoration, with sustained benefits for up to six months. However, both therapies are affected by methodological limitations, particularly in protocol standardization and patient selection. Preliminary studies suggest a synergistic effect when combining PRP and Li-ESWT, though further research is required.
Conclusion: Regenerative therapies such as PRP and Li-ESWT represent a promising paradigm shift in the management of ED, especially for patients unresponsive to pharmacologic treatments. Their widespread clinical adoption depends on the generation of high-quality, standardized, multicenter trials with extended follow-up periods and clearly defined eligibility criteria.
