Eficacia del examen pre-participativo deportivo en atletas menores de 35 años: historia clínica, examen físico y electrocardiograma

La muerte súbita en atletas jóvenes motiva el uso del examen pre-participativo deportivo (EPPD) para detectar cardiopatías silentes. Existe debate sobre si el electrocardiograma (ECG) debe sumarse a la historia clínica (HC) y el examen físico (EF). Se realizó una revisión sistemática en MEDLINE/PubMed sobre el rendimiento diagnóstico de HC, EF y ECG en menores de 35 años. La HC y el EF muestran sensibilidad baja (20% y 9%) con especificidad alta (94% y 97%); el ECG alcanza 94% de sensibilidad y 93% de especificidad, con la menor tasa de falsos positivos (6%). Los criterios internacionales de interpretación del ECG del deportista (2017) redujeron los falsos positivos sin perder sensibilidad. El ECG es el componente más eficaz del EPPD; su implementación requiere personal entrenado en su interpretación.Referencias
